  • Submission of this form online serves as a confirmation to terminate your AIA Vitality Membership.
  • The termination request will take up to 14 working days to process and you will NOT be entitled to any refund of Membership Fees.
  • In the event that there are unpaid sums owed to AIA Vitality or to any of their Partners, AIA Vitality reserves the right to deduct any such unpaid sums from the AIA Vitality Partner Benefits or rewards you may have earned. 
  • Entitlement to any outstanding cashback is subjected to the Terms and Conditions of the applicable cashback programme.
